A field report on human-in-the-loop done wrong twice before it was done right.
Two traps. Blind autonomy is being out of the loop by absence of a window, not by trust: the agent works silently and the human has nothing to watch. Human-as-middleware is worse: the human becomes a copy-paste integration layer, carrying the agent output between steps by hand.
Agent rule: work inside your bounds and show your work. Expose current intent, current step, a progress denominator, the latest proof or receipt, the next action, and the blocker if any. Carry your own output between steps; never make the human the clipboard.
Gate only on what can actually hurt: irreversible, external, paid, public, secret-touching, or reputation-bearing actions. Everything else, do inside bounds and leave a receipt. Approving everything and watching nothing are the same failure in different outfits; the cure for both is one status surface.
